Please find, below, the S4 and S5/6 Choice Form for next session as well as instructions on how to fill it in for each year group. This was issued to pupils at registration this morning.
GUIDELINES FOR THE COMPLETION OF THE S3 into S4 COURSE CHOICE FORM
Instructions on how to complete your choice form:
- You must choose English and Maths plus four other subjects.
- Once you have picked your subjects you must add the level of study (please refer to your prognosis report for this). Your parent/guardian should have a copy of this.
- Make sure you have discussed your choices with a parent/guardian and that they sign your choice form.
Please note the following:
- For the best possible chance of success in S4, it is strongly recommended that you continue with the study of the subjects you have studied in S3.
- If you have not studied a subject in S3 then you must speak to the Faculty Head and get them to sign your choice form.
- The level that you have been recommended may change depending on your classwork and effort.
- College courses count as a 7th subject and you miss a period of your other subjects on a Friday.
- Although every attempt will be made to meet your choices, some classes may not run if there is insufficient demand or if staffing is not available.
- Further information on each subject can be found in the school prospectus on the school website.
- Choice forms must be returned to the Main Office by Monday 28th March 2022.

- Make sure you have discussed your choices with a parent/guardian and that they sign your choice form.
Please note the following:
- For the best possible chance of success in S5, it is strongly recommended that you continue with the study of 5 of the subjects you have studied at National level in S4.
- If you have not studied a subject in S4 then you must speak to the Faculty Head and get them to sign your choice form.
- The level that you have been recommended may change depending on your classwork and effort.
- College courses count as a 6th subject and you miss a period of your other subjects on a Friday.
- Although every attempt will be made to meet your choices, some classes may not run if there is insufficient demand or if staffing is not available.
- Please note that while you are being asked to make choices about your courses, you may have to consider changes to your choices after your results. This will need to be discussed with your Guidance teacher at the start of the term in August.

- Once you have picked your subjects you must add the level of study (please refer to your prognosis report for this). Your parent/guardian should have a copy of this.
- Make sure you have discussed your choices with a parent/guardian and that they sign your choice form.
Please note the following:
- S6 students should take courses to meet their further or higher education aspirations and career/job intentions.
- If you have not studied a subject in S4 or S5 then you must speak to the Faculty Head and get them to sign your choice form.
- College courses count as an extra subject and you miss a period of your other subjects on a Friday.
- Although every attempt will be made to meet your choices, some classes may not run if there is insufficient demand or if staffing is not available.
- Please note that while you are being asked to make choices about your courses, you may have to consider changes to your choices after your results. This will need to be discussed with your Guidance teacher at the start of the term in August.
